Two breast surgery methods

Breast augmentation with implants is one of the best researched surgical methods. The implants used today are high-tech products of the highest quality - there is no more leakage and therefore the implant can remain in the body for a lifetime without any problems or harmful consequences. Implant manufacturers now even give a lifetime guarantee on many product features.

If the necessary conditions are met, a combination procedure (implants and autologous fat) can be considered in individual cases.

In most cases, breast augmentation with autologous fat alone (without implants) does not lead to a lasting satisfactory result. In addition, several operations are usually required. As a result, we only offer this surgical method (with autologous fat only) at the express request of the patient and not at a fixed price.

Highest quality implants

A basic distinction can be made between implants containing saline, hydrogel and silicone. We only use the most modern, high-quality implants filled with cohesive silicone gel.

The advantages: they cannot leak due to the strongly interwoven molecular chains, feel pleasant and soft to the touch (similar to biological breast tissue) and produce very beautiful, natural results.

We also offer breast augmentation with B-Lite implants, which are comparatively lighter in weight. However, the use of these implants is only suitable in selected cases and cannot be offered at a flat rate. If you are interested, please discuss this in your personal consultation.

Implant size

Our surgeons take a lot of time to understand your needs and your ideas in detail, whereby 3D simulation can also be used as an aid.

Round vs teardrop-shaped

Modern breast implants offer two proven shape options, each with their own advantages. The choice of the optimal shape depends on your individual anatomy, your aesthetic wishes and various medical factors.

Teardrop-shaped (anatomical) implants

Thanks to dimensionally stable silicone gel, these implants retain their natural teardrop shape in any position. They are particularly suitable if a soft, natural breast contour is desired and can create a very harmonious décolleté.

Round implants

This classic implant shape creates an even fullness and, depending on the initial situation and placement, can lead to beautiful, natural results. They are the ideal choice for certain body proportions and wishes.

The final shape is influenced by several factors:

  • The placement (above or below the pectoral muscle)

Ideal incision

The implant is inserted via the shortest possible incision, which is made in the breast crease beforehand. Great importance is attached to sterile insertion and at the same time to the shortest possible incision - so that the subsequent scar is as small as possible. Find out more here.

Placement methods

During breast augmentation, the implant can be placed in different body planes.

The aim of optimal implant placement is to place enough of the patient's own tissue over the implant. Only then can a natural and full breast shape be achieved. In addition, the risk of the implant being felt or the edges being seen is minimized.

If the patient has enough of her own tissue, we prefer subfascial placement: the implant is placed under the mammary gland with the muscle fascia taken along. This method offers decisive advantages over purely subglandular placement - the fascia provides better support for the implant and a more stable position.

If the patient does not have enough of her own tissue, the implant must be placed in deeper layers to counteract so-called "rippling" (visible rippling of the implant under the skin).

The dual-plane method offers the ideal solution here: the implant is placed partly under the pectoral muscle and partly under the mammary gland. This means that the implant is covered by the muscle in the décolleté area, while the lower part lies under the mammary gland. This produces stable and aesthetically pleasing results in the long term.

Requirements

Only a small amount of fat is required for this method. It is therefore also possible and helpful for slim patients.

Advantages

These combination procedures are being used more and more frequently because the breast volume and shape can be changed in a targeted and individualized manner.

There is often little autologous tissue in the lower part of the breast fold. The implant is therefore sometimes noticeable or even visible. Through the targeted use of autologous fat, these areas can be retouched by thickening and a very natural result can be achieved.

Particularly in thin women with little breast tissue, the visibility of unsightly implant edges can be prevented.

Existing asymmetries can be compensated for very precisely with the autologous fat. In addition, the autologous fat makes the skin softer and thicker, which means that the implant can be better covered.

An often appreciated side effect is that annoying fat deposits (such as on the abdomen, flanks, outer thighs, etc.) are suctioned out, which also improves the contour.

Placement

The implant is used for volume augmentation.
In most cases, the implant is covered in the upper area by the muscle (method: insertion of the implant under the pectoral muscle).

Procedure

The body's own fat is harvested from fat deposits, washed and specially prepared and injected into the surgical area after the implants have been inserted.

Personal support and aftercare

Immediately after surgery

Immediately after the operation, the breast is immobilized with a special breast bandage and evenly compressed. The higher pressure compared to a bra can reduce the risk of secondary bleeding.

You will remain in the day clinic for at least two hours for monitoring, after which you may be collected.

Follow-up appointments

Three follow-up appointments are necessary after the operation, the first of which takes place the very next day.

At this first check-up, you will be fitted with a sports bra, which should be worn consistently for six weeks. If a drain was placed during the operation, this is usually also removed the next day.

You will be informed promptly of the dates for the two further follow-up appointments.

Well-being

After breast surgery, swelling or temporary loss of sensation may occur, which in most cases disappears within a few weeks. However, it can take several months before the original sensation returns completely. After around six to eight weeks, the traces of the operation are usually largely healed.

Inability to work

You should expect to be unable to work for 3 - 4 days. Depending on your job, this may be longer.

Rest from sport

Sport must be completely avoided for 6 weeks, after which it is possible to return to sport more easily. Sports that put a lot of strain on the chest muscles, such as horse riding, strength training in the chest area or intensive yoga, should only be resumed after 3 months.

The risks of breast augmentation

  • Drug intolerance, bruising, wound healing disorders, inflammation, scarring problems or sensitivity disorders are examples of the risks that every operation entails. However, these always depend on the patient's individual state of health.
  • Another minor risk (3%) is the formation of capsular fibrosis. This is a natural foreign body reaction. If the tissue layer contracts, the implant is constricted, making the breast feel harder.
  • In severe cases of capsular fibrosis, corrective surgery is then performed to remove the scar tissue. This procedure is usually covered by health insurance. This is often done in combination with an implant change.
  • Furthermore, an implant may be displaced or twisted, resulting in deformation of the breast, which can lead to corrective surgery.

Grundsätzlich kann zwischen Kochsalz, Hydrogel und Silikon haltigen Implantaten unterschieden werden. Wir verwenden nur modernste, qualitativ hochwertige Implantate aus vernetztem Silikon. Die Vorteile: sie können nicht mehr auslaufen und ergeben sehr schöne, natürliche Resultate.

Nach der Operation ist die Konsistenz der Brust fester. Nur selten klagen Patientinnen während der ersten sechs Wochen über ein gewisses Fremdkörpergefühl. Im weiteren Verlauf wird die Brust weich und fühlt sich meist natürlich an. Bei sehr schlanken Patientinnen mit wenig Unterhautfettgewebe kann das Implantat vor allem seitlich aussen und im Unterpol der Brust tastbar sein.

Ein genereller Wechsel alle zehn Jahre, wie das früher empfohlen wurde, ist nicht nötig.
Wie bei anderen Eingriffen, wo mit Fremdmaterial und Implantaten gearbeitet wird, kann auch bei der Brustvergrösserung nicht ausgeschlossen werden, dass mit den Jahren eine Nachbehandlung erforderlich ist. Der Grund für eine allfällige Auswechslung ist in der Regel aber nicht die Haltbarkeit des Implantates, sondern häufig sind es Veränderungen des eigenen Körpers und Gewebereaktionen. So nimmt die Wahrscheinlichkeit eines Ersatzes mit zunehmendem Alter zu

Die Brustvergrösserung beeinträchtigt die Stillfähigkeit nicht, da das Implantat hinter der Milch produzierenden Drüse platziert wird.

Nein. Umfangreiche Studien zeigen, dass durch Brustimplantate kein erhöhtes Brustkrebsrisiko besteht.
